If you are going on vacation or will be away from
your residence or business for any period of time, please complete for
vacation notification form. This form will not only provide our
department with necessary information if anything happens while you are
away, but officers will also conduct periodic checks of your
residence/business. Print out and complete this form with as much
information as possible. When you are done, drop off or mail to
the police department. |

| Motor Vehicle Crash Form |
If you were an operator in a motor vehicle crash
in Massachusetts and the damage was over $1000 or someone was injured.
You must complete this form and mail a copy to the RMV and to the local
police department in the town/city you were involved in the crash at.
